New Client Intake Form

This form is required for all first-time clients. It provides important information about your health history, current concerns, and goals for your session. Completing this form helps ensure that your massage is both safe and tailored to your individual needs.

Minor Consent Form

Clients under the age of 18 must have a parent or legal guardian complete this consent form prior to receiving massage therapy. This form authorizes treatment and outlines expectations for minor sessions.

Energy Healing Consent Form

If you are receiving energy-based work (such as Reiki or other energy healing modalities), this consent form explains the nature of the session and confirms your understanding and permission to receive this type of work.
